When You Can Switch Plans
Option 1: Once Every 12 Months
Once in a 12-month period, you may change to any NYSHIP option for which you are eligible for
any reason.
Option 2: When You Have a Qualifying Life Event
You may change options more than once during a 12-month period only if you have a qualifying life event. In most cases, you must submit your change within 30 days.
Common life events include:
- A permanent move(a dependent moving to attend college does not count as a permanent move.)
- Marriage or domestic partnership
- Birth, adoption, or placement for adoption
You can find the full list of qualifying events in your General Information Book. Please note, the qualifying event must affect eligibility for health benefits, and a request for a change in health insurance deductions due to a qualifying event must be consistent with the event.
Check with your HBA or the BSC to find out if there are additional requirements or deadlines.

To review the different NYSHIP plan options available, you can compare your benefits, coverage and costs side by side online. Health Insurance Choices publication and the NYSHIP Rates flyer are also available for detailed plan information and premium rates.
How to Change Plans
If you decide to change your NYSHIP option:
- Complete the NYSHIP Health Insurance Transaction Form (PS-404R).
- Mail it to the Employee Benefits Division (EBD) as early as possible prior to the coverage effective date you are requesting. (The effective date must be the first of a month.)
In some situations, you may be subject to a late enrollment period. Speak with EBD for information about your specific situation.
