Medical/Surgical

With UnitedHealthcare's online directory, you can search for participating providers, laboratories, outpatient surgical locations, cardiac rehabilitation centers, urgent care centers and orthotic/prosthetic devices networks.

Hospital

Anthem Blue Cross is part of the largest hospital network in the U.S. with 97% of the nation's hospitals participating. Find a hospital, hospice or skilled nursing facility near you at their online directory.

Prescription Drug

Visit the CVS Caremark website to order mail service prescriptions online at reduced cost or find nearby retail pharmacy locations, including those participating in the vaccine network or with 24-hour service.

Nonparticipating Providers

If The Empire Plan is your primary insurance coverage and you use a nonparticipating provider who is part of the Basic Medical Provider Program, your out-of-pocket expenses may be reduced. Search for a provider or facility from the link below. 

Basic Medical Provider Program Directory

Welcome to The Empire Plan's online provider directory. With a few clicks, you can choose from a nationwide network of over 1 million providers, 15,000 hospitals, and 9,000 pharmacies. 

The Empire Plan is managed by four program administrators. This page links to their specific directories, each composed of the services they provide.

To start your search, simply click the desired program directory link and follow the prompts on the page to view a list of options near you.

The following descriptions will help you choose the right program directory to begin your search:

  • The Medical/Surgical Program is administered by United Healthcare. By clicking this link, you can find participating providers, laboratories, outpatient surgical locations, cardiac rehabilitation centers, physical rehabilitation providers and urgent care centers.
  • The Hospital Program is administered by Anthem Blue Cross. Visit this program directory to locate hospitals, hospices and skilled nursing facilities.
  • The Mental Health and Substance Use Program is administered by Carelon Behavioral Health. Click this link to identify participating providers and facilities, educational and screening materials and disease management programs. 
  • The Prescription Drug Program is administered by CVS Caremark. Go to their page to find pharmacy locations or order mail service prescriptions online at a reduced cost.
